Leila Pirnia
Founder & Clinical Director
I am a dually licensed psychotherapist in California specializing in ADHD, autism, AuDHD, giftedness, twice-exceptionality, and neurodiverse relationships. I work with couples who care deeply about one another but have become caught in painful patterns of miscommunication, shutdown, criticism, overwhelm, resentment, unequal responsibility, or emotional distance.
Many of the couples I see are intelligent, capable people who understand their diagnoses and may already know a great deal about neurodiversity. What they often cannot figure out is why that knowledge has not translated into a relationship that feels better. That is where I tend to work best.
My specialized training in neurodiverse individual and couples therapy includes extensive work with Dr. Harry Motro and Neurodiverse Couples Counseling Center, where I continue to practice as a clinician with autistic, ADHD, AuDHD, and otherwise neurodivergent adults and couples. I am also the Founder and Clinical Director of the Center for Twice Exceptional Counseling, where I work with gifted and twice-exceptional children, teens, adults, and families whose strengths and struggles often coexist in ways that are easy to misunderstand.
My approach combines a foundational understanding of neurodivergence with psychodynamic and attachment-oriented therapy, Internal Family Systems (IFS), family systems, CBT, ACT, DBT-informed work, and practical executive-function interventions. I am interested in both the surface problem and the meaning underneath it. A missed task may be about executive functioning, but after years of conflict it may also come to mean I can't rely on you or Nothing I do is ever enough.
Good neurodiverse couples therapy has to make room for both realities.
I don't believe either partner needs to become more neurotypical. I also don't believe that understanding neurodivergence means ignoring accountability, unmet needs, or years of accumulated hurt. My goal is to help couples understand what is actually happening between them, distinguish what needs accommodation from what needs repair or change, and find more effective ways to reach one another.
Ultimately, most couples are not coming to therapy simply because they want better communication skills. They want to feel close again. They want to feel loved, understood, appreciated, and secure with the person they chose. They want to be able to disagree without losing each other. That is the work I care most about.
Before becoming a psychotherapist, I spent many years in the corporate and technology world in leadership roles including president, CFO, COO, and founder. I earned my Bachelor of Science from MIT and later returned to school to earn a Master of Arts in Clinical Psychology from Pepperdine University. That earlier career continues to inform my work, particularly with highly analytical and high-achieving clients such as physicians, attorneys, engineers, founders, executives, academics, and other professionals who are accustomed to solving complex problems but find that intimate relationships require a very different kind of understanding.
Clinical Training, Experience & Credentials
Specialized Neurodiverse Couples Training & Clinical Work
I trained at and have worked extensively with Dr. Harry Motro and the Neurodiverse Couples Counseling Center, where I continue to practice as a clinician with autistic, ADHD, AuDHD, and otherwise neurodivergent adults and couples. My work there has focused on the ways neurodivergence can shape communication, attachment, emotional processing, executive functioning, intimacy, conflict, and long-standing relationship patterns.
Founder & Clinical Director, Center for Twice Exceptional Counseling
I am the Founder and Clinical Director of the Center for Twice Exceptional Counseling, where I work with gifted and twice-exceptional children, teens, adults, and families whose high cognitive abilities coexist with ADHD, autism, anxiety, executive-function challenges, emotional regulation difficulties, and other forms of neurodivergence. This work has deepened my understanding of the uneven profiles often seen in AuDHD and 2e individuals, where extraordinary strengths can exist alongside very real areas of difficulty.
Psychodynamic & Depth-Oriented Clinical Training
I completed clinical training at the Center for Professional Counseling of Los Angeles, with psychoanalysts Dr. Rachel Bar and Dr. Jack Mayhall. This depth-oriented foundation continues to inform my work with couples, particularly when present-day conflict has become intertwined with attachment patterns, family-of-origin experiences, protective defenses, and accumulated relationship injuries.
School-Based Mental Health Therapy
As a clinical trainee with Outreach Concern, I worked as a School-Based Mental Health Therapist serving children, teens, and families across multiple school sites, under the supervision of Dr. Anna Cho and Dr. Farrah Mizraee. My caseload included students experiencing behavioral, emotional, social, academic, and executive-function challenges.
Graduate Research Experience
I served as a Graduate Research Associate with Dr. Shelly Harrell in Pepperdine University’s Culture, Wisdom, and Resilience Lab, contributing to work focused on culturally responsive psychology, resilience, and wellbeing.

Our AuDHD clients learn how to:
These skills empower AuDHD couples to feel more connected, confident, and capable, whether they're navigating daily stressors, raising children, or managing careers. Through our work together, couples experience a shift from survival mode to meaningful connection, supported by strategies that actually fit their brains and relationship needs.
Our work is grounded in evidence-based modalities tailored for neurodivergent experiences. We view each person in the context of their environment, relationships, family, identity, and work, and offer tools that are actionable, affirming, and transformative. Therapy here isn’t about fixing what’s “wrong.” It’s about understanding what’s true and building a life that honors it.
This practice is informed by deep clinical experience and research in ADHD, autism, anxiety, OCD, executive functioning, and neurodiverse identity development. We specialize in working with high-achieving, emotionally intense individuals and couples, including those who may have been misdiagnosed or missed altogether by traditional systems. If you’ve always felt different, this is a place where you’ll feel seen.
